14th century, via Old French and Latin from Ancient Greek ἀγωνία (agōnía, “emulation, competition, struggle”), from ἀγών (agṓn, “contest”). Specifically of the struggle that precedes death (mortal agony) from the 1540s. By surface analysis, agon + -y.
