By surface analysis, hetero (“different”) + -onym (“name”). Compare Ancient Greek ἑτερώνυμος (heterṓnumos). First attested in 1885 in sense 1. The coining of this term in sense 3 (as Portuguese heterónimo) is usually attributed to Fernando Pessoa.
